What is required in order to schedule a new patient appointment?

 

  1. You must be registered as a patient at MGH. If you are new to MGH and need to register, please have your insurance information available and call: 1-866-211-6588.
  2. Please call our Brain Health coordinator, Sam Neveu, at the number above, to complete a brief phone consultation. After the consultation is complete, your responses and the records you send will be reviewed by our doctors. Sam will call you within 5 business days to set up your appointment.
  3. If you are being referred to us by a doctor outside of the Partners network, we suggest sending us the last 1-2 visit notes as well as any diagnostic testing relevant to your brain health. These notes can be mailed or faxed (not emailed). If you have had any brain imaging (CT Scan, MRI, etc.), please request the images on a disc for the appointment (if possible, please mail the disc to our address and notify us so we can expect the delivery and upload it into our medical record system prior to the appointment).

How do I know if my insurance will cover my visit?

To estimate insurance coverage, the Brain Health coordinator will ask you a series of questions during the initial phone consultation. Your responses will then be reviewed by our doctors along with any records that you send. The Brain Health coordinator will discuss insurance coverage with you at the time of appointment booking.
